IU Kokomo Semester Software Request
If you need assistance in completing this form, please contact the UITS-Kokomo Support Center at Ext. 9315 or send an e-mail to kohelp@iuk.edu.
This form must be completed and returned to the UITS-Kokomo Support Center by the following dates:
| Summer I and II => | March 5th |
| Fall Semster => | April 5th |
| Spring Semester => | October 5th |
The software and license must be delivered to the UITS-Kokomo Support Center by the following dates to allow for testing and timely deployment:
| Summer I and II => | March 30th |
| Fall Semster => | April 30th |
| Spring Semester => | October 30th |
