Student Activities Events Scheduling Form

This form is intended for those student organizations that are registered with Indiana University Kokomo's Office of Student Activities.  For more information, on the registration process, click here.  All non-registered groups should directly contact Event Planning.

Any and all events and meetings sponsored by a registered student organization occurring on campus (whether campus space is used or not) must be reserved and announced through submission of this form.  The completion of this form is only a request for the space reservation (if applicable) and is not an automatic or completed reservation until a confirmation is emailed to you; therefore, you mustn't advertise this event until the confirmation email has been received.  Please fill in all sections, otherwise your form may not be processed.

To serve food at your event, you must use the university's Food Services.  Only in rare cases, such as call out meetings, is this requirement waived.  For menu options, contact Student Activities at (765) 455-9203 before completing this form.  If you know that you would like catering at your event, but you are not sure of the menu or the headcount at this time, please indicate this in the space below and state when you will be able to provide the Office of Student Activities with your food order.
