Application Instructions

Applicants who are currently enrolled in prerequisite courses to satisfy the minimum number of credits may be admitted conditionally.

The Medical Imaging Technology program at Indiana University Kokomo has limited enrollment based on standards set by the Joint Review Committee on Education in Radiologic Technology. Admission to the program is very competitive. Applications to the program and/ or completion of the prerequisite courses do not guarantee admission into the professional portion of the program.

Application Process

Non-IU students should follow steps (1-6); IU students follow (3-6)

The Medical Imaging Technology program is a selective program.  It is important that you closely follow the application steps and requirements that follow:

1. Complete an application for Indiana University Kokomo in the Admissions Office.

2. Mail an official transcript from high school / GED scores and previous college to the Admissions Office.

3. Complete the required General Education prerequisites courses.

4. Send an application for the M.I.T. program to the Program’s Office.

5. Meet all physical requirements.

6. Complete the program questionnaire

7. Obtain three letters of recommendation.  One letter must be from a program official or other instructor.

  • Applicants not previously admitted to Indiana University should submit an admissions application to the Iindiana University Kokomo Admissions Office, supplying official transcripts from high school and post-secondary schools attended, and submit a Medical Imaging Technology application to the Allied Health Sciences Office, with copies of transcripts of all post-secondary education attached.
  • Applicants already admitted to Indiana University should submit a Medical Imaging Technology application to the Allied Health Sciences Office with copies of transcripts of all post-secondary education other than IU attached.
  • Applications are subject to review by the Radiologic Sciences faculty and selection committee. Medical Imaging Technology applications are available through the Allied Health Sciences office, Hunt Hall, Room 106.
  • Applications should be completed and returned with appropriate transcripts.  All applications and transcripts must be filed on or before March 1 of the year of anticipated admission (ex. March 1 of 2008 for admission in fall of 2008).

To be eligible for admission, each applicant must complete and return a completed questionnaire:

Step 1   Complete Medical Imaging Technology program application.

Step 2   Previous applicants circle and complete details on application

Step 3   Update all personal information on application

Step 4   List all previous and current institutions attended.  Submit a transcript for each college or university attended except for all campuses of  Indiana University .

Step 5   Prerequisite completion Plan – Applicants are required to answer (circle) either “YES" or “NO” to this statement.  Applicants answering “NO” must complete the prerequisite plan included within the Application Form

Step 6   Certification - All applicants are required to submit copies of their ARRT registry scores, ARRT certification with content section scores and any evidence of certification (ARRT, ARDMS etc).  Applicants are required to sign the application data sheet signifying they have read all instructions and understand the requirements necessary for their application to be considered by the program.

Step 7   Criminal History Disclosure –  Health care institutions may require a criminal history background check on all employees and persons who provide direct patient care.

You are required to send the program a criminal history disclosure report (see Instructions). Please include all criminal offenses (i.e. felonies and misdemeanors as well as non-criminal offenses.)

Applicants are not required to disclose such information on the application form.  Instead, please provide a typed statement of explanation, giving full details, including the facts and the disposition of the case(s).  This information should be submitted separately.  If such a previous criminal history exists, the school’s program director will confidentially discuss the information presented with the Division Chair.  This action is intended to ensure that the applicant’s previous criminal history will not cause the applicant difficulty in obtaining a licensure or certification in their intended core choice. The applicant may be required to contact certifying and licensure organizations for a letter of clarification before entry into the core courses of the Medical Imaging Technology degree program. The Radiologic Science programs reserves the right to require a full criminal history report be submitted by the applicant as part of the review process.

Applicants are responsible for maintaining a receipt certifying that the letter has been received by the Division of Allied Health Sciences. The cost of this screening is the responsibility of the applicant.

Applicants are responsible for maintaining a receipt certifying that the letter has been received by the Division of Allied Health Sciences.

Individuals interested in being admitted to one of the Radiologic Technology programs should contact the program of interest annually for an update of admission criteria.

  1. The application is revised each summer. Applicants must obtain an application for the year in which they wish to apply.
