LIMITED CRIMINAL HISTORY REPORT (LCHR) POLICY

The Division of Education requires all teacher education students enrolled in an education class with a field experience that requires direct or indirect contact with P-12 students to have a current LCHR on file in the DoE Office.   

                                Attention: To All Students Formally Admitted to TEP

To meet this requirement, students admitted formally into the Teacher Education Program (TEP) must provide a current Limited Criminal History Report to the DoE Office prior to the start of every semester.

                               Attention: To Students Not Yet Admitted    

                                                     

Students enrolled in the Education Learning Community, EDUC-F205/M101, P249/P251P255, and/or K205 must submit a current LCHR to the DoE Office.  For Fall and Spring Semesters, the LCHR must be submitted no later than Friday of the third week of class.   Due to the short summer sessions, the LCHR for Summer Semester classes must be submitted no later than Friday of the first week of classes.

Students who do not meet the appropriate deadline will automatically receive a Memo for Record (MFR).  As stated in the Policy Manual, “All MFRs issued to students or teacher candidates are subject to further evaluation by Education faculty at the Division of Education Benchmark Meetings.” (IU Kokomo Division of Education Student Policy Manual.)  Additionally, students without a current LCHR are not allowed to begin a field/practicum assignment or enter a school/childcare facility.

NOTE: In addition to the LCHR, all student names are cross-referenced with the Zachary Law database each semester.

Requesting a Paper or On-Line Report

You may obtain a Limited Criminal History Report by printing the paper version, completing the form, and mail as directed.

Note Regarding Reason for Request: Check Box #11: "Has volunteered services at a public school...."

Or, you may obtain an on-line Limited Criminal History Report by accessing the Indiana State Police website.


IMPORTANT!! You must have a credit card AND you must be at a computer that will print the form, or you will not be able to obtain the copy.

1. On the left hand side look for “Limited Criminal History” link and click it.


2. Read the following page and then answer the three questions on the bottom and hit “Search Now".


3. Regarding Reason for Request: Select #11.


4. You will be directed to print receipt for payment.  This is NOT YOUR REPORT.

5. Then, you must scroll to the top of the page.  Click "Get Report".  The criminal history form will appear on the screen for you to print out.  It should say "Does not reveal a criminal history report".

Submitting the Report to the Division of Education

1.  Make a copy for yourself.

2.  Take the original to the Education Office.
